That's the case in Illinois. My wife renewed hers last year. Her neurologist has to sign off on the forms.
My mom has had the temporary cards (that hang up from the rearview mirror) for several years but has had a HD (Handicapped Disabled?) plates on both vehicles since last summer.

Just renewed the one for the Toyota last month, this month is for the van. She still hangs her card in the rearview mirror (she's using it until it runs out.) But here in NC, not only does the doctor have to sign off on it, but the application has to sent back to the DMV (you pick them up at the local license plate office.)

Even if she forgets the card, it really doesn't matter: she's got the plate and all security has to do is check the back of the vehicle if they have any question.

And no, Wallyworld does NOT hand out those cards. Whoever said that was feeding you a load of BS.

Even Wal-Mart wouldn't be that stupid to do such a thing.

There is such a logic fail in this: They aren't going to give you a tag under any circumstances, but certainly not on the premise that you'd only be there for a short while. They want you to stay in the store for as long as possible and buy more stuff.

Yeah, I knew they were BSing me because I've been ticketing in our lots for 5-years. I KNOW what the stores do and don't, can and can't do. It just blew my MIND what they were saying, like they thought a mall cop would be THAT dumb!

Question: If you're issued a handicap plate/tag/whatever in State A, is it valid in the other 49 states?

IANAL, but I believe it is. Similar to how a driver's license in State A is considered valid in the rest of the country. But you still have to abide by local traffic and parking laws.

Yes, and several Canadian provinces. We looked into this when we planned to rent a car in California.

Felony stupidity.
In this day and time, Stupidity is still not considered an arrestable offense.

As we all know, SC's will use any and every excuse they can think up to try to weasel out of something.
