Yes I have. Not often though.

A few times it's because the employee has been awesome. Once it was because a guy in a kitchen had a large shaggy beard and no covering on it and we found a hair in our food (the owner was PISSED when I talked to him). And once or twice because the employee was horrid (one woman brought the check before bringing the food).
