The DOT really does hold truckers to a high standard. An acquaintance of mine was recently the victim of a "suicide by trucker" attempt. The 4-wheel driver waited at an intersection until the truck driver was far too close to stop then pulled out into the truck's travel lane and stopped. The trucker couldn't move to the left (vehicle passing him on that side) and didn't have much room on the right due to a narrow shoulder. He slowed down as best he could and aimed the truck so as much of the impact as possible was behind the driver. While he couldn't do anything at all to avoid the collision, he did everything he could to minimize injuries/fatality. Still, despite many witness statements that backed up the trucker's version of events, the initial ruling was that this driver could have prevented the collision. They eventually reversed that ruling, but it was a fight. (And the other driver survived. He might wish he hadn't have, but he did survive).
At the conclusion of an Irish wedding, the priest said "Everybody please hug the person who has made your life worth living. The bartender was nearly crushed to death.

You weren't kidding about being expected to be psychic... talk about impossible standards.

I'm told the FAA is the same way with planes - they always note how the pilot could have responded to whatever failure is in play. Not fun.
Don't forget that the "armchair quarterbacks" have hours to analyze the situation and pick an alternate course of action, while the guys "on the sharp end" have to decide in a split second.
Any fool can piss on the floor. It takes a talented SC to shit on the ceiling.
