I know there was a story about how the allies in WW2 found one of the nazi treasure vaults where they'd been hoarding stolen artwork/gold bullion and whatnot. They figured something valuable was inside because the place had a HUGE and obvious vault door made of inches-thick steel that they were mulling over how to blast open when a member of the engineer corps calmly strode up and blew a hole in the cheap masonry on either side of the door with a quarter stick of dynamite.
It's like in the Dresden Files books. Johnny Marcone, the top mob boss in Chicago, has dealt with Harry numerous times, and has all doors and entry points made from cheap materials that will disintegrate easily, so that when Harry barges in to demand to know what Marcone knows about something, there will be less cost in repairs, and less debris to cause collateral.

But all dynamic entry points (the walls next to doors, etc.) are reinforced to prevent "heroics."
