Just something to think about: constrained, pre-chosen security questions are how Sarah Palin's Yahoo account was hacked. When faced with these I provide nonsensical answers, to eliminate the chance that someone could gain access simply by looking up a few facts.
I just use Password Safe to store my passwords, and to minimize the time they spend "in the clear" -- browsers don't encrypt stored passwords at all unless you set up a master password.
I use SFSP (PDF) for my most important passwords. Makes them easy to remember while being quite secure. Only problem is it tends to make them long, often exceeding the maximum limits. Doh!
