I work as a file clerk for a medical imaging facility... Mainly I make sure that doctor's offices get reports when they call for them, other facilities or patients get films if they need them, and pull mammogram films for tests being done in the next few days. I rarely deal with patients that are actually having the tests that day, so the techs will get information from a patient on where their last test was done.
Had a patient on my screen that wasn't read from the previous day, so I check. She lists her previous study as 'Memorial Imaging (Center)'. Now... maybe that's us, maybe it's not, no one in this down with that name. So I look and... nope, not with us, not with either of the other hospitals we're part of. So... time to check with the patient.
Thankfully, because the techs can check this stuff and SHOULD check this stuff before the patient walks out the door, I can hand this kind of thing to them to take care of... but I decided to stand there while she calls the patient.

Tech - mammo tech that did the patient
PT - Patient who is clueless but claims not to be
Tech: Hello, Mrs. Patient. This is Tech who did your mammo yesterday. We're trying to find your previous films and they were not done here.
PT: I know where my last ones were done!
Tech: Well, I'm looking at the form you filled out and it says 'Memorial Imaging', but we don't have a record of you having one done here.
PT: I didn't write that on there.
Tech: (looks at the form again) Ma'am, right next to 'Where was your last mammogram done?' it says 'Memorial Imaging Center'.
PT: *pause* Well, I don't have that in front of me to verify that.
PT: Then it must have been at <imaging center right behind us>.
Tech: No, Ma'am, our file clerk already called there and they don't have a record of you having been there for anything.
PT: I know where I've been.
Tech: (rolling her eyes) I understand that, ma'am, but...
PT: Then it was <imaging center down the road from you>.
Tech: Well, ma'am, I called there and they don't have you on file, either.
PT: Well, I did have a follow up exam at <doctors who also do mammograms>.
Tech: Ok, ma'am, we'll check with them then, thank you.
It never ceases to amaze me how people will insist they KNOW where they were and then send you on a wild goose chase.
Had a phone call from one of the other facilities today looking for films. At least their patient was honest with them...
"Well, I think it might have been at <imaging across from them>, but I'm not really sure, so you'll just have to call around to find them."
And then they call and complain that their test hasn't been read in over a week.
