So I'm the new trainer at work, I should know better than this and I should have been more observant....

I missed a crucial step in process when I didn't make sure that my machine was running correctly last night. It kept prompting me that all of my product was good.

It made it all the way to the next department where it was discovered to be wrong.

All because I didn't pay attention. Yeah, and I'm the trainer.........

It probably would've been the same thing for me if the machine kept saying the product was fine. In this case, the machine was more at fault for giving you wrong information.

Last night I was told that it was an engineering problem.

Aka, sometime that day, engineers were screwing around with it and never replaced the software. So nope, not my fault!

Are you sure you were just verifying that those further down the line were paying attention and didn't need any extra training. (sounds like a good answer to me)
