Sorry to hear the manager was spineless; at that point if I hadn't already ordered, I probably would have told him either that family goes, or my family goes, and either way a letter would be written to the company. (In fact a letter may be warranted regardless now).

At the Parking lot threat, while I don't carry, I would have taken out my phone and said I was calling the cops to have him arrested for making threats. But the cops would probably have a hard time arresting the cartoon dust cloud he would have left trying to get out.

This is why I'm happy that the places I tend to frequent are local, family-run places where everybody knows everybody. I think if anyone tried that crap at the place I go to for breakfast, they'd get shanked by half the patrons with butter knives.
