Yeah, it's noticeable, but doesn't make it hard to understand me.
So, I had to make a call for one of our clients, we reached a person who was rocking an accept that makes Jackie Chan sound clear and concise (I'm not sure how I would define the accent, but needless to say, really thick).
So the cast is me and the stupid hearing user (SHU for short).
Me: This is [relay] assisting a deaf or hard of hearing person placing a phone call, are you familiar with how relay calls work?
SHU: I sorry, your accent is too thick for me to understand.
Me: I'm assisting a deaf or hard of hearing person placing a phone call, are you familiar with how these calls work? (said slowly and clearly)
SHU: Sorry, I still don't understand, let me transfer you to someone who is better with understanding difficult callers like you.
