I'm going to add my vote here for the manager being an absolute jerk for not reacting quickly enough and getting her away from you. There is absolutely NO reason whatsoever for anyone to treat anyone that way, be it customer, coworker, family, whatever. I'm hoping our words can help you move on from this, because there's nothing else we can do at this point. Welcome to the site! This place is great. Even I've cried over a customer before..though that scared the managers so bad they had the customer escorted out .. quickly. Because when I start crying, means I am getting angry. Angry + Customer = bad.

First off, you did nothing wrong, and at least you're not THAT douchebag (nor anything close to it). You don't want to grow up and be like her.

Secondly, I wonder what would have happened had you simply held her item and prevented her from getting it until she apologized. Sure, it would piss her off more, and it would make the manager come over sooner as she'd be flipping her lid.

How hard would it be to have all cashiers refuse her service when she comes in? Additionally, I wonder if it would be justified in calling 911 if she refuses to leave the premises next time as instructed (on the grounds of harassment and disturbance of the peace).

your manager failed, he should have never let that harpy anywhere near you. she never said what you did wrong, did he even ask her what you did that offended her? your other customer didn't want to hang around, volunteering to run after another item instead of getting the staff to get the item? your manager should have seen that as a clue, that this insane b!tch is creating a scene and can't be good for the other customers or for business. he should have handled this at customer service or taken it outside. there was no need for her to come back to you and abuse you like that.
