This is from a few years ago, but I won't ever forget this.
I went through the drive-thru of the local Bell and placed my order. When I finished, the person on the other end of the line asked if I wanted my last item prepared as vegetarian. I made a WTF face, said no, and pulled around.
When I did, I asked her why she asked me that. She said, "I noticed that everything else you ordered was essentially vegetarian, so I thought maybe you didn't know the last item had meat in it."
Freakin' awesome. Someone actually paid attention to what I ordered, put two thoughts together, and gave enough of a crap about me as a customer to make sure I got something I'd be happy with. I called the corporate help line and heaped praise on her and the store in general--I've never had bad service there and never had to wait long regardless of the number of customers ahead of me.
THAT, ladies and gents, is the way a fast food place should be run.
