The toilets at my office have indicators on the outside. You can tell at a glance (from 10 feet or so away) whether they're in use or not. And the light switches are on the inside.
There's a good reason for putting light switches on the outside, though: electrical safety. There are approved types of switch for use in bathrooms, but most of these weren't widely available until a decade or so ago. Putting a normal light switch in a wet environment is just asking for trouble.

Of course, we wait long enough to be sure our victim is firmly planted on the toilet. After all, we want to avoid the possibility of a slip & fall type injury.
