The toilets at my office have indicators on the outside. You can tell at a glance (from 10 feet or so away) whether they're in use or not. And the light switches are on the inside.

There's a good reason for putting light switches on the outside, though: electrical safety. There are approved types of switch for use in bathrooms, but most of these weren't widely available until a decade or so ago. Putting a normal light switch in a wet environment is just asking for trouble.

I'm kind of guilty of that.

Our mens room has one stall and one urinal. When I use the urinal and leave I reflexively turn off the lights. Sometimes there is someone in the stall when I do this. They usually just shout and I turn the lights back on, but it is still embarrassing.

After she starts to come out, without washing her hands, of course, she proceeds to tell me (in a very pissy tone while displaying a glorious catbutt face) that the toilet is stopped up, and was that way when she went in there.
She used it, knowing it was stopped up?!?!?

I get sick just thinking what these people's houses look like.
Sorry, my cow died so I don't need your bull

It sounds like a sadistic boss had installed the light switch on the out side. Or else likes scaring customers right before the store closes.
We don't even have a switch in our bathrooms at work. We kill the lights by flipping the breaker. ... in the electrical box, which is conveniently located well away from the bathrooms. 's comon practice (when there are no customers around) to wait for a fellow employee to go in there, then run to the breaker box and simultaneously flick the breakers marked "restrm lits" and "restrm fans", thus simulating a blackout. Of course, we wait long enough to be sure our victim is firmly planted on the toilet. After all, we want to avoid the possibility of a slip & fall type injury.

I can't even begin to form a coherent response to any of that! I would very much like to attempt to have a conversation with this person to try to figure out just what they could possibly have been thinking, but I don't think I can do that, not because I have no way to find this person, but because I would endanger my own mental health!
I wouldn't try engaging in conversation with her. It was the head of HR, so you're not likely to accomplish anything by conversing.

No offense to anyone on here who is in HR, but in my experience, most HR staff are blithering idiots. Which may explain my OP...
