Just thought of another one to add, this from my mom's experience as a teacher's aide when we'd moved to Phoenix -
I forget what the teacher's actual last name was, just that it began with an "S" and was German in origin.....was one of those names that you'd have to see written a few times before you'd remember the spelling. Anyhow, apparently the students had to fill out some kind of paper (and write their teacher's name), and as my mom was going over these papers, she came across one where the student had spelled the teacher's name as "Mrs. Shitdick".

I have a name associated with a popular movie. So a lot of times someone will read my nametag and say it multiple times, or ask if I'm related to the character from the movie, or say a popular line from the movie, or ask me to say a popular line from the movie. I used to go to school with a girl named Sunshine Hawaii. Oddly enough, she wasn't a very cheerful person.
My maiden name when I was born was Knight. My father wanted to name me Stormy or Wednesday but my mother threatened his life. If they had named me Wednesday though I never would have married my first husband. His last name was Adams.

In the other direction, I came across a doc with a rather shorter name. I had a funny situation with this one; I'd just scribbled down a phone script from a Doctor Williams, and I wrote "W'ms" on the paper, because that's how you abbreviate William and I was in a hurry. My tech, who was typing, had not heard of this abbreviation before and I had to elaborate. Then the aforementioned doctor called in, and I handed her a paper with "Oo" written in the Doctor field. She asked "So what does that stand for then?" I had to tell her that it was his whole name, it doesn't stand for anything. She didn't believe me, and I told her I wasn't kidding, he was already in our computer and everything. (His first name was Zaw, by the way. If I had to guess I'd say he was probably from Burma, except they spell it "U" there...)
Oh, and once I came across a Doctor The. No kidding. I had to wonder if he was related to Doctor Who...
(Edit: There's an ER doctor in Suffern, NY whose name is Rasch. I told him once that with that name he ought to have been a dermatologist.)

I had some guests in the hotel one time whose names were Mr & Mrs Drunk.
They were actually a nice, sober, elderly german couple.
My highschool had a shop teacher named Mr. Wood, and a gym teacher named Mr. Ball.
