Here's the response I got from Sam's Club, plus the letter I sent them. The letter explains my grievance.

I want you to know that we are reviewing the tapes as I type this email and the operator of the forklift will be held accountable. This is in no way the way we operate our business. I want to thank you for bringing it to our attention and I want you to know that all forklift operators will be addressed. You mentioned not having spotters and this too will be addressed. I will personally ensure that this issue is taken care of. Again I apoligize for you not feeling safe while shopping with us.

Just taking some time to tell you how close I, my five year old daughter, and several other people came to getting killed in your XXXX location in South Carolina today.
The answer is Pretty close. In my particular case, about three feet or so. 
This incident Im going to tell you about happened roughly between three and four in the afternoon, possibly a little later. Its important to understand the store was pretty heavily crowded. So theres really no excuse for this incident.
I was walking up one of the two main corridors that lead up to the front of the store, pushing a very heavy cart with a bad wheel. Riding in this cart was my five year old daughter. There was a woman walking ahead of me. The cart stuck for a moment, breaking my stride, and when I faltered in my step, a fork lift came tearing out of a side aisle (about where the office supplies are, I think), speedingand I do mean SPEEDING between me and the woman I was walking behind. If I had not paused to get a better grip on the cart I was pushingand I only hesitated for an instant the forklift would have crashed right into me and my daughter at a high speed.
It tore down the next aisle at the same speed, not even pausing, and I assume made its way down to the other side of the store.
The beeping alarm had been sounding. However, it had been sounding steadily all over the store for the last twenty minutes of my visit. It had been sounding off and on for the entirety of my visit, and I was probably in there for a total of an hour and half. If you cant see it, its impossible to tell where it is as the sound bounces and echoes. I am hard of hearing, and that does not help. I do look around as Im in the storeI know forklifts are in use, and I am aware of my surroundings. But pushing a cart down a main corridor, where you cant see over the shelves, I dont really expect to have to worry about being run over by a high speed vehicle.
Does your company not employ spotters? Ive worked at places myself where heavy equipment had to moved through crowds. We had people looking out for public safety. We sure as heck didnt drive things at high rates of speed across blind intersections on main corridors. Ive gone into other stores where they dont even let their customers down aisles where theyre moving heavy equipment. Yet, your company allows forklifts to play Speed Racer in crowded stores?
As a parent, I have enough to worry about without having to worry about my kid getting run over by a speeding vehicle inside a building. Needless to say, Im pretty freaked out. Probably not going to be back inside that particular store. I just cant risk it, if you dont have any sort of safety protocol. I hope the Harbison store is better, but you can bet Ill be finding out before I bring my kid in there. Can you imagine if the forklift had broadsided my cart with her in there? I hope you can, because right now, I cant imagine anything else. Although I sure wish I could. I doubt Im going to get any sleep tonight. It makes me want to vomit just thinking about it.
I did look around for a manager, but I didnt see anyone to lodge a complaint with. Im certainly not going to take it out on the cashier. So I figured Id look on your site.
This may come as a huge shock to you all, but if you kill someone because you are playing fast and loose with public safety, youre going to end up embroiled in a big, bloody, lawsuit. The irresponsibility I witnessed today was beyond the pale. Shameful.

That was a close call for sure! If they act right they will fire that douchebag.
